A Community Health Nurse providing advanced clinical home care services including patient assessment, medication management, and education to support recovery from illness or surgery. Responsible for coordinating and evaluating individualized care plans while collaborating with patients, families, and caregivers in a home setting. Requires nursing licensure, experience in medical-surgical or home health, and strong clinical decision-making skills within a not-for-profit health system.
Fee for Service rates:
Initial & ROC Weekday visit rate: $100
Inital & ROC Weekend/Holiday visit rate: $120
Revisit daily rate: $85
Orientation daily rate: $250
In-Service hourly rate: $55
Weekday and weekend shifts available.
